COVID-19 Vaccination Information

Individuals 12 years of age or older who live in Florida may receive the COVID-19 vaccine according to Florida Health. Minors aged 12 to 17 years of age may receive the Pfizer vaccine but must be accompanied by a parent or guardian.
RESIDENCY REQUIREMENTS
All patients must provide proof of Florida residency in order to receive the vaccine at any site.
Part-time residents (snowbirds)
Part-time residents are eligible to receive a vaccine in Florida. A part-time resident is any person who temporarily resides in this state for a period of at least 31 consecutive days in each calendar year, maintains a temporary residence in this state, returns to the state or jurisdiction of his or her residence at least one time during each calendar year, and is registered to vote or pays income tax in another state or jurisdiction.
What do you need to prove Florida residency?
A Florida state ID or Florida driver’s license will suffice alone. If you do not have a Florida ID, you must prove your residency with documentation. Examples of proof of residency include:
- A deed, mortgage, monthly mortgage statement, mortgage payment booklet or residential rental or lease agreement.
- One proof of residential address from the seasonal resident’s parent, stepparent, legal guardian or other person with whom the seasonal resident resides and a statement from the person with whom the seasonal resident resides stating that the seasonal resident does reside with him or her.
- A utility hookup or work order
- A utility bill, not more than 2 months old
- Mail from a financial institution, including checking, savings, or investment account statements, not more than 2 months old.
- Mail from a federal, state, county, or municipal government agency, not more than 2 months old.

If you are deaf or hard of hearing, you can call 711 (Florida Relay service or email 311@miamidade.gov. For individuals 12 to 17 years of age may receive the Pfizer vaccine at one of the County sites and must be accompanied by a parent or guardian and must bring a completed parental consent form.

Winn-Dixie/Fresco y Más recommends printing and filling out the consent form before your appointment (forms will also be available in stores).

Teens and young adults aged 12 to 21 who reside in Florida can be vaccinated at this site. This site is open for appointments and walk-ups 7 a.m. to 6:30 p.m. Tuesdays and Thursdays, and 9 a.m. to 1:30 p.m. Saturdays at the Nicklaus Children’s main campus.
